Rheumatoid arthritis needs an aggressive early management of the disease. In the USA the long term effects of this syndrome appear to have the most disabling and expensive incidence on quality of life. At the American College of Rheumatology Annual Scientific Meeting 2003 the new, aggressive strategies of treatment, called DMARDS ( biological disease modifying antirheumatic drugs) were evaluated. The papers present all the new strategies, with recently FDA approved antirheumatic products, also in support of biological safety. The data suggest that combination therapy with TNF (tumor necrosis factor) inhibitors and MTX ( methotrexate) provides greater benefits in improving both the symptoms and the signs of the disease. The activity of these drugs has been evaluated in the control of radiographic deterioration and in the improvement of vital signs. Further appropriate studies must be conducted, but, so far, the data suggest a careful balance between risks and benefits.

Two of the most important causes of discomfort and stress in the American population are rheumatoid arthritis (RA) and ankylosing spondylitis (AS). Both conditions are responsible for the loss of a high number of work-days and for a reduced quality of life in patients aged 16-59.
1056 patients with rheumatoid arthritis and 658 with AS were included in this study. The patients with rheumatoid arthritis showed a worse physical health related quality of life but a better mental health. Somatic pain, physical role functioning, social functioning, emotional role functioning, vitality, or general health perception were similar in the two groups.
A very interesting finding was about work which was found to have a positive association with physical health related quality of life in both groups.
